In order to proceed at the opening of
your file, simply print , fill in and sign all three forms.
Forward by mail to ACDM office with the required documents.

Being Always listening to our members, we have decided to soften the methods of payment for the membership fee. You can now pay your membership in two simple ways:

- The membership cost is $150, payable in total or 3 equal payments of 50$ with Visa, MasterCard or American Express.
- In order to open your file (75$)
This payment is non-refundable. - If your school is not in our recommended school list, you need to add 75$ for your practical evaluation
This payment is non-refundable.
Important: Hypnotherapist has necessarily need to make a practical evaluation - Proof of registration in a course at an accredited school.
MEMBERSHIP FEES
- Active Member 150$
- Inactive Member 50$
- Student Member 25$ (which is credited when admitted as an active member)
- Support Member 50$
Opening of file: 75$
The fees for the opening of
your file are valid for one year. After this delay, if you did not
become a member, you will have to pay anew the fees in order to
keep your file active.
Membership fees: 150$
You must forward copies of your diplomas ,attestations of marks, resume, and two photos in passport format (passport size). Also please send completed Admission Forms.
However, the ACTMD reserves the right to require an practical evaluation to be taken if we feel more credibility is needed after studying your file.
IMPORTANT
- A discount on the next renewal of the membership fee will be offered to every member who will refer a person. As soon as the adhesion will be ratified, the member referring will receive a 25$ certificate on his next renewal.
- A member can refer many potential members and receive more than one discount until the completion of the amount of the annual fee, a possible saving of 150$ a year.
